I. Purpose

This policy defines how Direct Primary Care of Coconut Grove verifies clinician licensure, confirms patient location prior to provision of telemedicine services, and ensures prescriptions are issued only where legally permissible. The policy safeguards patient safety, compliance, and integrity of prescribing.

II. Practice Model & Provider Identity

Direct Primary Care of Coconut Grove is a solo direct-primary-care and telemedicine practice. Sole clinician/owner: Camilo Andrés Murillo, APRN, FNP-BC. There are no additional providers. The practice is not a marketplace or multi-provider platform. HIPAA-compliant EMR (Elation Health) is used. Payments are collected only for services rendered directly by the clinician.

IV. Patient Location Verification

Patients must provide their current physical address and state of residence at scheduling and confirm this at the start of each telemedicine encounter. Care is provided only if the patient is physically located in a state where the provider holds an active license. The EMR documents patient location and provider attestation. If a patient is located outside of a licensed jurisdiction, the visit does not proceed.

V. Prescribing Policy

Prescriptions are issued only when the patient is located in a state where the provider is licensed. All prescriptions are transmitted exclusively to licensed retail or mail-order pharmacies. Prescriptions are never dispensed directly by the provider. The EMR logs all prescribing details and blocks out-of-jurisdiction prescribing attempts. Rationale, indication, and counseling are documented.

VI. Controlled Substances Policy

No controlled substances are prescribed under any circumstances to any patient, in any state or jurisdiction. Controlled substances require a direct in-person physical examination and cannot be prescribed via telemedicine by this practice. Patients requiring controlled substances are referred to local in-person providers, urgent care centers, or emergency departments.

VII. Pharmacy Interaction & Verification

All prescriptions are routed to licensed pharmacies. Pharmacies verify patient eligibility and location in accordance with state and federal law. Patients are advised that prescriptions are only valid through licensed pharmacies in states where the provider is authorized to practice.

VIII. Consent, Documentation & Audit

Patients sign telehealth consent acknowledging the limitations of telemedicine, including that no controlled substances will be prescribed. All records are securely retained in the HIPAA-compliant EMR and are available for audit or regulatory review.

IX. Exceptions & Referrals

Patients located outside of licensed jurisdictions are not prescribed medications. When immediate treatment requiring controlled substances is clinically indicated, patients are referred to in-person providers. Urgent or emergency needs are directed to the nearest emergency department or urgent care.

X. Compliance with Stripe Requirements

This policy ensures that all payments processed through Stripe represent direct professional services provided by one licensed clinician (Camilo Andrés Murillo, APRN, FNP-BC). No prescriptions are dispensed or sold through the website. Prescriptions are only issued through licensed pharmacies. No controlled substances are prescribed under any circumstances. Patient location and licensure compliance are strictly enforced.

XI. Responsibility & Review

The provider is solely responsible for maintaining licensure and compliance with state and federal law. This policy is reviewed annually and updated as needed. Any future staff will be trained in these protocols to ensure strict compliance.

XII. Signature / Attestation

I, Camilo Andrés Murillo, APRN, FNP-BC (NPI: 1245642180), attest that I am the sole clinician providing services under Direct Primary Care of Coconut Grove. I affirm that I will not provide prescriptions to patients outside my licensed states, that no controlled substances will be prescribed under any circumstances, and that all prescriptions are issued only through licensed pharmacies in compliance with law.
